Privacy Policy

The Community Partnership's Privacy Policy describes how our agency manages and collects personal information of persons served through our programs. The policy describes the Partnership’s use of both hard copy and electronic data with specific emphasis on the management and collection practices of the DC Homeless Management information System (HMIS) which the Partnership administers on behalf of the DC Continuum of Care.

Attachment -Continuum of Care Privacy Notice
The Continuum of Care Privacy Notice is a uniformed posting that all programs operating the Homeless Management Information System must post. This notice is required to be posted at the intake area of any shelter or housing program. The notice explains the reason an agency asks for personal information. No person will be denied services for refusing to allow their personal information to be entered in the HMIS.

Attachment - Provider Privacy Template
The Provider Privacy Template serves as a guide to instruct agencies on how to write a Privacy Policy. It addresses the entire basic subject areas required of agencies outlined in the HUD Data and Technical Standards.
