Innovative Programs

Chronic Homeless Initiative

The Chronic Homeless Initiative was a pilot program started with HUD funding in 2004. The program provides permanent housing to chronically homeless individuals with mental illness and co–occurring substance abusedisorders who are living on the streets or in the Federal City Shelter. Chronically Homeless community partnership - Project Hopepersons are defined by HUD as unaccompanied individuals with a disabling condition who are homeless for one year.

This program provides housing as the first intervention and later incorporates wraparound supportive services to help individuals maintain their housing.  Through successful partnerships with Pathways to Housing D.C. and Green Door and other homeless provides the program has successfully housed 131 individuals.
